EMPLOYEE BENEFITS

Full-Time employees (defined as working an average of at least 30 hours per week) are eligible for the following benefits on the 1st of the month following 60 days of employment.

Medical Insurance

• Choice among 2 PPO plans or HDHP with H.S.A. Option

Dental Insurance

• 100% for preventive services, 80% for basic services, 50% for major services; 50% for orthodontics services to age 19. 

Vision Insurance

• Up to $100 reimbursement for exam, contacts, frames per 12 month period.

Life Insurance

• Company paid term life insurance ranging from $10,000 to $50,000. Employee-paid voluntary life insurance available for employee, spouse and/or dependents.

Short-Term Disability Insurance

• Employees are eligible to purchase up to 66 2/3% of weekly earnings to maximum of $1,000 per week; 26 week maximum duration; 14 day waiting period.

401(k) Plan

• Employees are eligible to enroll after 1 year of employment with a discretionary company match and are immediately vested upon enrollment.
